Our physicians are handy to aid any person in the area who is confronting a serious or life-limiting illness. Our specially trained, advanced practice nurses and board-certified doctors provide management to all of our clinical programs and work with our hospice patients.
They band together with the patient’s current physician as a complement of the care patients are already being given, respond to inquiries, control pain and other symptoms, and help patients live well through severe illness.
Our physicians and advanced practice nurses are available to visit patients where they live—whether at home, a nursing home or assisted living facility—for consultation and follow-up visits.
Care includes:
- Assisting patients to manage pain and other symptoms, such as fatigue, nausea, shortness of breath, loss of appetite, constipation or difficulty sleeping.
- Being with patients and families during all of the trials that are part of dealing with serious illness.
- Helping with planning for future treatment, such as helping institute the most appropriate care setting.
- Evaluating the patient’s medical information and working with patients and their relatives to consider treatment options so that they can make the choices that suit their concerns and objectives.
Services are billed to your medical insurance. Medicare, Medicaid and private insurance are accepted.
